Aneurysms Clipping or Coiling or Stenting
A brain aneurysm is a balloon-like bulge that develops in a weakened area of an artery wall due to thinning of the blood vessel. This may compress and damage surrounding brain tissue. An aneurysm can also rupture, causing blood to leak into the surrounding tissues. This haemorrhage can damage brain tissue and increase pressure inside the skull, disrupting the brain’s oxygen supply.
Aneurysms can be caused by a range of factors, including high blood pressure, ageing, family history, head injury, and atherosclerosis (hardening of the arteries). They can lead to serious complications, including stroke, loss of consciousness, and, in severe cases, death.
Symptoms
Unruptured brain aneurysms often cause no symptoms. However, some may cause pain around the eye, visual disturbances, a drooping eyelid, or weakness, numbness, or paralysis on one side of the face. The most common symptom of a ruptured aneurysm is a sudden, severe headache. Other symptoms may include nausea and vomiting, a stiff neck, confusion, blurred vision, loss of consciousness, and seizures.
Diagnosis and Treatment Options
Aneurysms are typically diagnosed using magnetic resonance imaging (MRI), often with gadolinium contrast, and cerebral angiography. These investigations allow specialists to obtain detailed three-dimensional images of the aneurysm. Depending on its size, location, and structure, an aneurysm may be treated with clipping, coiling, or stenting.
Treatment depends on the size, location, and characteristics of the aneurysm. Small aneurysms may be monitored closely. Medications may be used to relieve symptoms, manage pain, and help prevent complications such as seizures, stroke, or vasospasm (narrowing of blood vessels).
Common treatment options include:
- Microvascular Clipping Microvascular clipping involves placing a metal clip across the neck of the aneurysm to stop blood flow into the aneurysm and prevent bleeding. If the affected artery is damaged, a bypass procedure using a vessel graft may be required. This procedure involves opening the skull (craniotomy).
- Endovascular Coiling (Embolisation) Endovascular coiling is a minimally invasive procedure in which a catheter is inserted through an artery in the groin and guided to the aneurysm using X-ray imaging. Tiny metallic coils are then deployed into the aneurysm, promoting blood clotting within the aneurysm and preventing further blood flow into it.
- Endovascular Stenting In some cases, a stent may be placed within the affected blood vessel to support the artery wall and redirect blood flow away from the aneurysm. Stenting is often performed in conjunction with coiling for certain aneurysm types.
Recovery
Recovery time for patients with an unruptured aneurysm is generally shorter than for those who require emergency treatment for a ruptured aneurysm. Recovery varies from patient to patient and depends on the size and location of the aneurysm, the type of treatment performed, and the patient’s overall health.
